Governor Rendell has announced that $1 million from the Recovery & Reinvestment Act has been awarded to Pennsylvania for programs that prevent and manage chronic diseases in older adults. The ‘Help Yourself to Health’ chronic disease self-management program includes four PA area agencies on aging (AAAs) – PCA, Allegheny County, Berks County, and Cambria County. PCA will work with 12 community-based organizations, including senior centers and non-profit agencies, to train professionals and recruit 1,600 older adults to participate in a six-week educational program. The program is expected to serve 3,000 older adults state-wide by assisting them in preventing and managing conditions, such as diabetes, high blood pressure, heart disease and arthritis; and thus preventing hospitalizations and long-term care.
